BRING COMPLETE SET OF APPLICATION REQUIREMENTS:
- CSC exam application form (CS Form No. 100) - you may download it from the CSC website

- Valid ID (original and photocopy; ID should bear the date of birth)
- four pieces passport size photos (prescribed CSC format)
- PHP500.00 examination fee
NOTE: Acceptance of applications shall be on a FIRST-COME, FIRST-SERVED basis and shall be closed any time before the deadline once the target number of applicants is reached.
