** Please follow the processes below for the application of your Passport. **
STEP 1: REGISTRATION and APPLICATION PROCESS:
1 Please bring all the required documents (original and photocopies) on July 19-21, 2022 at the PAG-ASA Sports Complex. *LIMITED SLOTS ONLY! The Registration date may end earlier once all the slots are filled.*
2 Fill out the Passport Application Form and sign the waiver. (These will be provided in the Registration Site.)
3 Have the requirements checked by the LGU – Representatives.
4 Wait for the LGU – Representatives to encode all your details in the DFA website.
5 Check carefully that all your encoded personal information is correct.
6 Once done, proceed to Step 2.
STEP 2: APPLICATION CONFIRMATION PROCESS:
1 Please check your provided email address for the Reference Number and Payment Details immediately after the application. (EMAIL SUBJECT: DFA Passport Appointment System – Payment Reference)
2 Pay the amount of P1,250.00 (including convenience fee) in the DFA Authorized Bayad Centers only within 72 hours upon receipt of the email.
3 An Appointment Confirmation mail will be received once paid. (EMAIL SUBJECT: DFA Passport Appointment System – Confirmation Notification)
4 Follow the instructions provided in the email.
i. Print all the attached documents of the 2nd email in A4-size paper
ii. Bring the printed application form, e-receipt, and checklist together with all the supporting documents on the appointment date (August 4, 2022)
STEP 3: APPOINTMENT AND APPEARANCE PROCESS:
1 Applicants must arrive 15 minutes before their appointment at the PAG-ASA Sports Complex on August 4, 2022.
2 All applicants must bring the original and photocopies of ALL the requirements during the POW Appearance Date on August 4, 2022 (include supporting documents, if applicable).
IMPORTANT REMINDERS:
1. Applicants must bring all the requirements during the POW Application and Appearance Dates;
2. Make sure to provide an ACTIVE EMAIL ADDRESS;
3. Processing Fee is NOT Refundable;
4. All issued passports from the POW activity will be delivered via courier services. The applicant must pay an additional P150.00 per application on the appearance date in the DFA courier partner.
5. Non-compliance with the requirements will result to the rejection of their application and forfeiture of their payment;
6. Applicants with lost and mutilated Passports will not be entertained;
7. Applicants with undeclared lost Passport will be rejected;
8. Applicants with missing, incomplete, and/or discrepant entries in their documents will be rejected;
9. Applicants without their application packets will not be processed on August 4, 2022;
• Application packet includes (i) checklist and process guide, (ii) completed application form and (iii) two (2) copies of eReceipt
10. Strictly NO WALK-INS on August 4, 2022 including all priority individuals;
11. In case of No Show during the appearance date, the processing fee will not be refunded.
For Senior Citizens, Person with Disabilities, those minor children seven (7) years old and below, and other priority individuals, please note:

2 All priority individuals will have a separate lane during the Application Dates (July 19-21)
For minor children below 18 years old, please note:
1 The mother must accompany the minor child in applying the passport. Please bring the original and photocopy of one (1) acceptable valid ID of the mother.
2 If the mother can’t accompany the child:
- If the parents are married and the father will accompany the child, bring the parent’s Marriage Certificate and one (1) acceptable valid ID of either parent
- If the parents are unmarried, a Special Power of Attorney (SPA) indicating “for passport purposes only and the minor will be travelling with the mother” executed by the mother is required and photocopy of one (1) acceptable valid ID of mother and representative.
- If the Special Power of Attorney (SPA) is executed abroad, it must also include Affidavit of Support and Consent with name of the minor’s travelling companion which are both authenticated by the Philippine Embassy or Consulate.

• Government Service Insurance System (GSIS) Card
• Unified Multi-Purpose Identification (UMID) Card
• Land Transportation Office (LTO) Driver’s License
* Student Permit may be accepted if in card format.
• Professional Regulatory Commission (PRC) ID
• Philippine Identification (PhilID)
• Overseas Workers Welfare Administration (OWWA) E-Card
• Commission on Elections (COMELEC) Voter's ID or Voter's Certification issued from the COMELEC Main Office in Intramuros, Manila
* Voter’s Certificates issued in COMELEC’s regional/local offices must be supported by a NBI or police clearance.
• Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
• Senior Citizen ID
• Airman License (issued August 2016 onwards)
• Philippine Postal ID (issued November 2016 onwards)
• Seafarer’s Record Book (SRB) by the Maritime Industry Authority (MARINA) (issued February 2020 onwards)
• Valid or Latest Passport (For Renewal of Passport)
• For minor applicants (8 years old and above), School ID with 2020-2021 or 2021-2022 validation or Certificate of Enrolment issued by the school with minor’s photo and school’s dry seal overlapping the photo
• For applicants based overseas, they may use their host government issued IDs showing their Philippine citizenship. (Example: Residence Card)
Important Note:
The Department may require additional supporting documents as may be necessary.
Reminders:
1. Acceptable Valid IDs must have entries (i.e. name and date of birth) that are consistent with the entries on the PSA birth certificate.
2. All IDs presented should be readable, untampered and contains consistent information with the documents presented upon application.
3. Original and photocopy of IDs and supporting documents are required to be presented upon application.
