All elected SK officials (Chairperson and Kagawad) must undergo the SKMT before they can assume office, pursuant to Section 28 of the Republic Act No. 10742, otherwise known as the Sangguniang Kabataan Reform Act of 2015, as amended by RA No. 11768.

1. Certificate of Proclamation from COMELEC
2. ID
3. Registration Fee receipt
