Qualified students of San Jose City aged 17-30 years may now apply at the Public Employment Service Office (PESO), 2nd Floor City Hall Building.

Requirements:
1. White, long folder with fastener
2. 6 pcs passport-size pictures with name tag (signature over printed full name)
3. 2 pcs photocopy of birth certificate (from Local Civil Registry or PSA)
4. 2 pcs photocopy of valid ID of parents/beneficiary (bring original valid ID for verification)
5. 2 pcs photocopy of grades (bring original copy for verification):
• College level – 1st semester grades SY 2022-2023
• Vocational level – Certificate of Completion
• High School level – Form 138 (2nd or 3rd grading period SY 2022-2023)
• Out of School Youth – grades for last school year attended

*No dropped, incomplete, or failed grades

6. Certificate of Tax Exemption (from BIR Talavera, if parent/guardian is unemployed)
7. ITR Taxable2022 (if the parent is employed, government/private)
The combined income (NET after tax) of parents should not exceed PHP189,504.00
